  • Do I need an appointment?
    Appointments are preffered for non-emergency new patients. Scheduling an appointment is as easy as making a simple phone call (260-482-1551) or sending an email (MTDentalGroup@outlook.com).
  • Which insurance plans are you in network with?
    We are currently in network with the following insurance plans: - Parkview Signature Care - Delta Dental Premier - Anthem 300 - Cigna
  • What information should I bring for my first visit?
    For your first visit, please have the following on-hand: - Dental insurance (if applicable) - List of current prescriptions - Knowledge of past and current medical history For more information regarding your first visit, please visit our New Patients page to download all necessary forms.   • I have a dental emergency. What number should I call?
    In the instance of a life-threatening emergency, please dial 911. For a non-life-threatening dental emergency, please call us at (260) 482-1551.
